Checklist for Personal and Business Profiles


 

         

 

If you feel we can be of help to you, than use this checklist in gathering the information needed to start the process.         


ü     Indicate your form of business: Sole Prop., C or S Corp, LLC (if LLC, indicate if taxed as S or Partnership).


 

 


 

ü     List the number of owners, % of ownership, and how related (spouse, child, etc).


 

 


 

ü     Are there other businesses you have ownership in? If so, please name, and provide information as indicated before.


 

 


 

ü     Provide three years of annual financial statements, including P & L Statement, Balance Sheet and, if available, Statement of Cash Flow.


 

 


 

ü     Tax returns for the most recent three years, including adjustments, for business and personal.


 

 


 

ü     A schedule of all compensation, salary, and distributions to owners and officers of the company.


 

 


 

ü     A census of all employees of the company (employee name; date of hire; date of birth; position/job description; total annualized W2 compensation, broken down by base pay, bonus, commissions, etc ).


 

 


 

ü     Listing of assets by type, value, and ownership, to include real and residential property, investments, IRA’s and other retirement plans (copies of statements welcome).


 

 


 

ü     Originals or copies of individual life, disability, retirement, and group medical plan policies or summary plan descriptions.


 

 


 

ü     Information on any owner or key person selective benefit plan in place.


 

 


 

ü       Copies of current will, business operating agreement, buy-sell, and related documents.
